Job Description:
A company is looking for a Care Navigator.
Key Responsibilities
Provides outreach services to various member populations,
coordinating service and linkage to benefits and resources
Assesses and evaluates members' needs to guide them towards optimal
wellness and facilitates interactions with appropriate
resources
Develops relationships with community resources and implements
interventions for self-management and access to health and social
services
Required Qualifications
Bachelor's degree in social services, behavioral health, or related
field, or equivalent combination of education and experience
2 years of experience in care coordination activities, preferably
in a health plan/managed care or clinical setting
Experience working with populations that have complex
biopsychosocial needs
Certification in Care Coordination or other industry-recognized
certification preferred
Intermediate Microsoft Office skills and knowledge of community
resources and medical terminology
